Insurance and Billing

I am able to bill the following insurances directly:

• Fidelis

• Excellus BC/BS

• Some other BC/BS plans

• United Healthcare

• MVP

• NYS Empire Plan

Your insurance provider may reimburse you for services. Some have a billing form I can fill out. Some require you pay for services out of pocket and submit receipts of payment. Please contact me for my provider number before you call your insurance company.

I CANNOT work with CDPHP or Horizon.

If you are paying directly, I can accept the following methods of payment:

• Cash

• Check

• Credit Card

• HSA Cards

I operate on a sliding fee scale for private pay services.   Please contact me for more information about rates.
